Duplicate documents on education: diploma (bachelor, specialist, master) is prepared and ordered in the Ministry of Education and Science of Ukraine by the educational department of GIIM SHEI "DDPU"
To obtain a duplicate diploma (bachelor, specialist, master)
the following documents must be submitted:
- Written statement of the owner of the document addressed to the director of GIIM SHEI "DDPU". The statement shall state the reason for loss or damage of the document, surname, name, patronymic, address, telephone number of the owner, name of the lost or damaged document, full name of the educational institution and date of its completion, other information if the owner considers them essential for the issuance of a duplicate.
If the graduate does not have information about the series and number of the diploma and other details of the lost diploma, it is necessary to apply to SE "Inforkesurs" for an extract from the register of educational documents (Kyiv, O. Dovzhenko str., 3, tel. (044 ) 290-18-14
2. Certificate from the police (militia), which indicates under what circumstances, where and when the loss of the diploma occurred. The certificate must be properly executed (signature, seal).
3. Announcement in the print media at the place of residence of the holder of the educational document, which must indicate the name of the educational document, its number and date of issue, in whose name it was issued, by which educational institution and its invalidation.
4. A copy of the payment document on payment of the costs of making a duplicate of the document on education.
5. Consent to the collection and processing of personal data.
6. Photocopy of passport, 1-2 pages in duplicate.
7. Photocopy of passport - one copy.
8. Photocopy of the original document (if available)

Duplicate applications are made according to the form (sample) valid on the date of issuance of the duplicate. If the document does not correspond to the form (sample) valid on the date of issuance of the duplicate, the applicant is issued an archival certificate in Ukrainian.
