List of documents for a social scholarship

Sample statement

To write an application for a social scholarship, you must have the following list of documents:

  • Passport (and extract if ID)
  • Identification code
  • Student ticket (or certificate from the place of study)

Also, according to the category under which you fit, you must provide:

1. For orphans and children deprived of parental care, and persons from among them in the case of continuing education until the age of 23 or before graduating from educational institutions:

- a copy of the decision of the guardianship and guardianship authority to grant the status of an orphan child or a child deprived of parental care, or an extract from the accounting and statistical card of an orphan child or a child deprived of parental care;

2. Individuals who were left without parents during their studies at the age of 18 to 23 (whose parents died/declared dead, died or went missing) (one of the documents):

- a certificate issued by a state-owned enterprise entrusted with the functions of the National Information Bureau in accordance with Law of Ukraine "On the Defense of Ukraine", according to the form established by the Ministry of Reintegration, which contains information about parents who are prisoners of war (held captive by the aggressor state)/deprived of personal freedom (detained, taken hostage) by the authorities of the aggressor state (occupation administrations and armed forces formations);

- a document confirming the death of the parents, issued by a military formation, a special law enforcement agency;

- a copy of the following documents:

death certificates of parents;

- the court's decision to declare the parents dead;

- court decision on recognition of parents as missing/excerpt from the Unified Register of Persons Disappeared Under Special Circumstances (before its operation - extract from the Unified Register of Pre-Trial Investigations), certifying the fact that the parents are wanted as missing under special circumstances or recognized as missing under special circumstances.

If a person was raised by a single mother, an extract from the State Register of Civil Status Acts of Citizens about the state registration of a child’s birth, issued by the Department of State Registration of Civil Status Acts, or a birth certificate issued by the executive body of a village, township, or city council, indicating the reason for entering information about the child's father to the act record of the child's birth in accordance with paragraph one the first part of Article 135 of the Family Code of Ukraine;

3. Persons who according to Law of Ukraine "On the status and social protection of citizens who suffered as a result of the Chernobyl disaster" have benefits during the appointment of a scholarship:

- a copy of the appropriate certificate confirming the status of a person who suffered as a result of the Chernobyl disaster, or a copy of a certificate of the prescribed format, issued to a minor child of a deceased citizen, classified in category 1, 2 or 3 participants in the liquidation of the consequences of the accident at the Chernobyl NPP, whose death is connected with Chernobyl disaster;

4. Miners who have at least three years of underground work experience in accordance with Article 5 of the Law of Ukraine "On Increasing the Prestige of Mining Labor"

And also persons who became students within three years after obtaining basic and/or full general secondary education, whose parents are miners, who have at least 15 years of underground work experience or who died as a result of an accident at work or who have been diagnosed with disability I or II group, in accordance with Article 5 of the Law of Ukraine "On Increasing the Prestige of Mining Labor":

  • certificate from the mining enterprise indicating the experience of underground work;
  •  act on an accident related to production, according to the H-1 form, which was valid until July 1, 2019;
  • a copy of the medical and social examination certificate;
  • a copy of the father's (mother's) death certificate.

If it is not possible to submit a certificate from the mining enterprise indicating the experience of underground work, a certificate from the Pension Fund of Ukraine indicating the experience of underground work is submitted;

5. Persons recognized as injured participants of the Revolution of Dignity, participants in hostilities in accordance with the Law of Ukraine "On the Status of War Veterans, Guarantees of Their Social Protection", and their children (until the children finish their studies at an educational institution, but not longer than until they reach the age of 23 ), - copies of the following documents:

– the certificate of the injured participant of the Revolution of Dignity of the established model;

- certificate of a participant in hostilities of the established model;

- certificates of the person's direct participation in the anti-terrorist operation, ensuring its conduct and protection of the independence, sovereignty and territorial integrity of Ukraine, certificates of the person's participation in the implementation of measures to ensure national security and defense, repel and deter armed aggression of the Russian Federation in the Donetsk and Luhansk regions, ensuring their implementation or a certificate of a person's direct participation in measures necessary to ensure the defense of Ukraine, the protection of the safety of the population and the interests of the state in connection with the military aggression of the Russian Federation against Ukraine, in the form in accordance with applications 14 and 6 to the Procedure for Granting and Depriving the Status of a Participant in Combat Actions to Persons Who Defended the Independence, Sovereignty, and Territorial Integrity of Ukraine and Directly Participated in an Anti-Terrorist Operation, Ensuring Its Conduct, or Taking Measures to Ensure National Security and Defense, Repelling and Deterring Armed Aggression of the Russian Federation in Donetsk and Luhansk regions, ensuring their implementation, in the measures necessary to ensure the defense of Ukraine, the protection of the safety of the population and the interests of the state in connection with the military aggression of the Russian Federation against Ukraine, approved by the resolution of the Cabinet of Ministers of Ukraine dated August 20, 2014 No. 413;

6. Persons who have been deprived of personal liberty as a result of armed aggression against Ukraine, after their release; children of persons who have been deprived of their personal liberty as a result of armed aggression against Ukraine, as well as children of persons who have been deprived of their personal liberty as a result of armed aggression against Ukraine, who have died/died (before such children finish their studies at the relevant educational institution, but no longer than until they reach 23 years old):

 - an extract from the Unified Register of persons in respect of whom the fact of deprivation of personal liberty has been established as a result of armed aggression against Ukraine, in the form approved by the Ministry of Reintegration, or a copy of the decision (protocol) on establishing the fact of deprivation of personal liberty as a result of armed aggression against Ukraine;

Children of the above persons:

- an extract from the Unified Register of persons in respect of whom the fact of deprivation of personal liberty has been established as a result of armed aggression against Ukraine, in the form approved by the Ministry of Reintegration, or a copy of the decision (protocol) on establishing the fact of deprivation of personal liberty as a result of armed aggression against Ukraine;

– a copy of a document confirming a family relationship with a person who has been deprived of personal liberty as a result of armed aggression against Ukraine (birth certificate, court decision on recognition of paternity) (as of January 1, 2024);

7. For children, one of whose parents was killed during mass civil protest actions or died as a result of injury, contusion or mutilation received during mass civil protest actions (before the end of educational institutions, but no longer than before they reach 23 years old), copies such documents:

- death certificates of father (mother);

- certificates or certificates of a family member of a deceased (deceased) war veteran of the prescribed format;

8. For children registered as internally displaced persons (until the end of their studies at an educational institution, but no longer than until they reach the age of 23):

a copy of the certificate of registration of an internally displaced person under the age of 18.

9. Persons to whom social scholarships of the Verkhovna Rada of Ukraine for students of higher educational institutions from among orphans and children from low-income families are assigned:

a copy of the order of the Cabinet of Ministers of Ukraine on the appointment of a social scholarship of the Verkhovna Rada of Ukraine to students of higher educational institutions from among orphans and children from low-income families;

10. Children with disabilities and persons with disabilities of I-III groups:

– a copy of a medical report on a disabled child under the age of 18 or a copy of a medical and social examination certificate;

11. Students from families receiving aid in accordance with Law of Ukraine "On state social assistance to low-income families":

- a certificate from the social protection authority of the population on the appointment of family assistance in accordance with Law of Ukraine "On state social assistance to low-income families".

12. Persons with disabilities due to war in accordance with Law of Ukraine "On the status of war veterans, guarantees of their social protection" and their children (before the children finish their studies at an educational institution, but no longer than before they reach the age of 23):

- a copy of an identity card with a disability as a result of the war of the established model;

13. To the children of deceased, deceased (missing) participants in the Revolution of Dignity, participants in hostilities, persons with disabilities as a result of the war or Defenders of Ukraine (before the children finish their studies at an educational institution, but no longer than until they reach the age of 23) , - copies of the following documents:

- death certificates of father (mother);

- certificates or certificates of a family member of the deceased/certificate of a family member of the deceased Defender or Defender of Ukraine of the established model;

14. To children who live in settlements on the contact line (until the children finish their studies at an educational institution, but no longer than until they reach the age of 23),

- an extract from the register of the territorial community, which contains information about the declared/registered place of residence (stay) of the person in the settlement on the demarcation line.
